You will need to provide the following information when you apply for a Blue Badge:
National Insurance Number
Your National Insurance number (if you have one).
Contact details
Your contact details (for example, phone number).
Email address
An email address is required if you wish to apply online yourself as we will email you with updates about the application.
Proof of address
Dated within the last 12 months:
- proof of address
- consent to check your personal details on our Council Tax, Benefits or Electoral Register database
- if applying on behalf of an under 16 year old we need to see the Child Benefit documents showing their address
Proof of identity
One of the following must be submitted with the application:
- valid passport (if you have a new style passport you must provide a copy of the page which displays your photo, name and passport effective and expiry dates), EEA identity card or UK residence permit
- valid driving licence or Valid Bus Pass (with photo)
- birth or Adoption or Marriage or Divorce certificate
- civil partnership or dissolution certificate
- National Insurance number card or medical card
- GP patient medical summary or history letter
Photograph
A recent passport standard photograph showing face and shoulders is required:
- children’s photo’s aged six and over must be passport standard showing face and shoulders
- children photo’s aged five and under must show a clear image that is a true likeness of the child
- nothing else should be in the photo and no one else should be in the photo
Proof of eligibility
Dated or issued in the last 12 months if the applicant:
- is severely sight impaired or Registered Blind - a Certificate of Vision Impairment (CVI) signed by a Consultant Ophthalmologist which states you are Certified Blind or Severely sight impaired. If you don't have a CVI you can confirm on the application form that you give us authority to confirm with the Visual Impairment Team that you are Registered Blind or Severely sight impaired and provide proof of your Registered Blind or Severely sight impaired card
- is in receipt of the Higher Rate of the Mobility Component of Disability Living Allowance - letter of entitlement or an annual uprating letter (Phone DWP on 0800 121 4600 or 0800 731 0122 to obtain a new letter if you don’t have one)
- is in receipt of the “Moving Around” descriptor for the Mobility Component of Personal Independence Payment, that is, score 8, 10 or 12 points and letter of entitlement showing this. You can provide a previous award letter as long as you also provide your Annual uprating letter (dated or issued in the last 12 months). (Phone DWP on 0800 121 4433 to obtain a new letter displaying the points if you don’t have one)
- is in receipt of the Mobility Component of Personal Independence Payment and has obtained 10 points specifically for Descriptor E under the “planning and following journeys” activity, on the grounds that you are unable to undertake any journey because it would cause you overwhelming psychological distress and letter of entitlement showing this. You can provide a previous award letter showing this as long as you also provide your Annual uprating letter (dated or issued in the last 12 months). - is in receipt of War Pensioner’s Mobility Supplement - letter of entitlement (Phone DWP on 0800 169 22 77 to obtain a new letter if you don’t have one)
- is in receipt of Armed Forces and Reserve Forces (Compensation) Scheme award - letter of entitlement confirming receipt of tariffs 1-8, which also certifies that you have a permanent and substantial disability which causes inability to walk or very considerable difficulty walking (Phone 0800 169 22 77 to obtain a new letter if you don’t have one).
- drives a specially adapted vehicle due to a disability in both arms - a copy of their insurance details
- is a child under 3 - Healthcare Professional letter giving details of medical condition and type of medical equipment needed
Your medical condition or disability
- what is your medical condition or disability (for example, details relating to your medical condition, diagnosis or disability)
- confirmation of how your disability affects you whilst walking
- information regarding any coping strategies you may use and how effective they are
- a letter of diagnosis, as up-to-date as possible (that is, a GP patient medical summary or history letter)
- evidence of the progression of the condition over time
Mobility aids
Which mobility aids do you use (for example, wheelchairs, scooters, zimmer frames or crutches).
Health care professionals
- the names and contact details of any health and social care practitioners involved in your diagnosis and ongoing treatment
- provide supporting documentation confirming the health or care professionals involved in the treatment of your condition (that is, GP or hospital appointment letters)
- evidence of specialist consultations, or referral for such
- GP patient medical summary or history letter
- Education Health and Care Plans (EHCP)
- care plans from social care teams
- social housing letters or assessment reports from a local authority
- contact details of professionals who can support your application
Treatments
- which surgeries, treatments, or clinics you attend or have attended for your condition
- details of treatments you receive to help you manage your condition
- confirmation of ongoing treatments, clinic attendances, or referral for such
Medications
- which medicines or pain relief do you currently take for your condition, that is, prescription or written list of medicines (you do not need to obtain a GP letter)
- details of medication(s) you receive to help you manage your condition, that is, prescription or written list of medicines (you do not need to obtain a GP letter)
Terminally ill applicants
When applying you need to provide the DS1500/SR1 form which contains details of diagnosis.
If the original DS1500/SR1 isn’t available a photocopy is acceptable or a letter from a medical professional confirming the existence of the DS1500/SR1.
The DS1500/SR1 can also be used for Proof of ID and Proof of Address or No payment or photo is required.
